Please include as much information about the violation as you can. The following details are especially helpful: (1) the date/time of the violation, (2) the location of the violation, and (3) the names and positions of the people involved in the violation. In many cases, the information that you supply will be the only information we have about the violation. Our FAQ page discusses several common scenarios and has questions that will show what kind of information is most useful for each one.

Because our lawyers may need to gather additional details about the violation you are reporting, it is important that you provide us a way to get in touch with you—otherwise, we may not be able to take any action on your complaint. We will keep your identity strictly confidential – we will not reveal your identity or share your contact information without your express permission.
